Dr Rosenzweig focuses on relieving symptoms and supporting wellbeing in ways that blend natural therapies with standard medical practice. Office hours. Office hours are offered most Mondays. We are able to schedule telemedicine visits on other days if the patient is located in Pennsylvania at the time of the visit. In order to schedule you for your first appointment, we first need to receive your intake packet.

Complete the intake packet ahead of time. Download the Intake Packet from this page. Or request by email from staff@stevenrosenzweigmd.net  Please return the packet by fax (888-802-0516) or email (staff@stevenrosenzweigmd.net).  If you are a patient within the Penn, Jefferson, Main Line Health, Temple, or Fox Chase systems, you can give us consent on these forms to access your electronic medical record. If that is the case, there are many fields on the health history form that can be left blank. If you receive your care through some other provider, we'll need a recent visit note from your doctor and ourIntake Packet contains a medical release form for you to give your doctor's office. This is a visit summary that is written by your doctor at the end of the visit that includes your medical history, exam, test results, and treatment plan. That note can be faxed to our office at 888-802-0516.
