Your First Visit

What to bring to the first visit

Please complete the registration form and medical intake form ahead of time. They can be downloaded below, or our office can mail them to you. We can't start your appointment unless these forms are completed, and this may result in a shortened visit.

Bring key medical records to the first visit. In general, these would include:
--Relevant lab work, imaging studies, or other test results relevant to the consultation.
--Consultant reports that are relevant to the consultation

Cancer patients are asked to send or bring:
--Copy of your treating oncologist's most recent progress note
--Copy of blood test results in the last 3-6 months
--Copy of report of latest imaging studies (you do not need to bring the DVD)
--Pathology reports. (NO SLIDES PLEASE)
